Support Broker Services, LLC is assisting a participant in the iCare Partnership program to recruit a dedicated Direct Care Professional. Please note: This position is not employed by Support Broker Services, LLC. The successful candidate will be directly hired by the participant through iCare Partnership.
About the Role:
This is a unique and fulfilling opportunity to provide one-on-one support that empowers an individual to live independently and with dignity. Your work will directly enhance their quality of life.
Key Responsibilities:
- Assist with activities of daily living (ADLs) including bathing, dressing, grooming, toileting, and mobility/transferring.
- Provide medication reminders and support with health-related tasks as delegated.
- Prepare nutritious meals and assist with feeding if needed.
- Perform light housekeeping tasks such as laundry, dishwashing, and tidying living areas.
- Accompany the participant to medical appointments, community activities, or errands.
- Document daily progress, observations, and any changes in condition.
- Communicate respectfully and effectively with the participant, their family, and the ICare Partnership support team.
Qualifications:
- High school diploma or equivalent preferred.
- Previous direct care, CNA, or home health aide experience a plus.
- Valid driver’s license and reliable transportation (if required by participant).
- Ability to pass a background check as required by Wisconsin law.
- Compassionate, patient, reliable, and respectful of participant choice and privacy.
