RN – Private-Duty Home Health (1:1 In-Home Setting)

We are seeking a dedicated Wisconsin-licensed RN to provide specialized, long-term home care for an adult with complex medical needs. The Details:

· Pay: $25.00 - $35.75 per hour (set through a participant-directed budget).

· Location: Private residence (consistent commute).

· Schedule: 12-hour shifts.

What You'll Do:

· Provide expert, personalized care management and proactive behavioral support.

· Implement person-centered care plans in collaboration with the family and Support Broker Services.

· Educate and empower family members and support staff to ensure continuity of care.

· Ensure the dignity, safety, and independence of your patient in their own home.

Who You Are:

· A Wisconsin-licensed RN with a passion for home health or complex one-on-one care.

· A critical thinker who thrives working autonomously in a home environment.

· Able to meet Wisconsin DHS caregiver standards (including background checks).
